We welcome new patients to our practice & look forward to serving your healthcare needs. We accept all major medical plans along with most Medicare plans. We are not contracted with Medicaid & are unable to accept this as a primary or secondary insurance.

Physician referrals are needed if required by your insurance (HMO plans) It is the responsibility of the patient to obtain & maintain an active & current referral. We will request the referral as a courtesy, but ultimately the patient is responsible for making sure they have an active referral on file.
Medical records are required prior to any appointment to expedite your treatment plan. Please contact any physicians that have treated you previously to have your records faxed to us at (214) 948-7701. Please include any office visit notes, procedure/surgical notes, physical therapy notes & imaging reports.
You will need to bring picture ID, insurance cards & current medication list to ALL appointments.
Copays, deductibles, coinsurance & balances are due at the time of service. If you are unable to pay & have not made payment arrangements your appointment will be canceled & rescheduled.
If you need to cancel or change your appointment, we require 24 hours advanced notice of your scheduled time. If you cancel with less than 24 hours-notice or miss your appointment there is a $25.00 fee assessed.
