Out of courtesy to our patients, we do our best to remain on schedule throughout the day. For your convenience, our new patients forms are available to be printed and completed, at your convenience, prior to your appointment. Filling out the forms prior to your appointment allows for more time with the physicians and less time in the waiting room.

If you do not have the forms completed prior to your appointment time we may have to reschedule your appointment.

If you have difficulty printing the forms, please contact our office at (214) 948-7700 so we may mail or fax you a copy.

If you have any Xrays, MRIs or medical reports, please bring them to your appointment or have them faxed to (214) 948-7701.

Please bring your current insurance and picture ID with you to every appointment.
